Summary

*

The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

• Travel to multiple locations to provide nursing and condition specific education to your patients in their homes in a time efficient manner.
• Provide admission, case management, and follow-up visits for patients.
• Administer on-going care for each patient, provide necessary follow-up as directed by the DON.
• Confer with physician in developing the initial plan of treatment based on physician’s orders and initial patient assessment.
• Render hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under DON's supervision.
• Report patient care/condition/progress to patient's physician and DON on a continuous basis.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in Illinois and CPR certification.
• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.
• Computer skills adequate for data entry of clinical documentation.
• Current Illinois Driver’s License and proof of current auto insurance.

Most of our patients reside in Kane, DuPage and DeKalb counties. We can work with a weekend nurse to find an area that works for them.

Pay: $55.00 - $120.00 per hour
