Requirements
**
- Graduate of an accredited Practical Nursing program.
- Current Licensed Practical Nurse (LPN) license in the state of practice.
- Current BLS certification preferred or required per facility policy.
- Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.

Responsibilities
**
- Provide direct nursing care in accordance with individualized care plans.
- Administer medications and treatments as prescribed.
- Monitor, assess, and report changes in patient condition to the Registered Nurse or provider.
- Assist with admissions, transfers, discharges, and ongoing patient care.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, nurses, and the interdisciplinary healthcare team.
- Educate patients and families within the scope of practice.
- Follow infection prevention, patient safety, HIPAA, and regulatory standards.
- Perform other duties as assigned.
