You'll play a key role in helping patients transition successfully through rehabilitation while working with a team that values coordination, consistency, and long-term impact.
*What You'll Do*
- Partner with a PM&R physician to manage patient care in skilled nursing and sub-acute rehab settings
- Collaborate closely with therapy teams (PT/OT/ST) to align medical care with functional recovery goals
- Coordinate with primary care providers, nursing staff, social workers, and families to ensure continuity of care
- Provide ongoing evaluation and management with a focus on pain control, mobility improvement, and reducing hospital readmissions
- Complete timely and accurate documentation, with flexibility to chart remotely

*What We're Looking For*
- Licensed Nurse Practitioner or Physician Assistant (required)
- Active certification as an NP or PA (required)
- Current Michigan license (required)
- Ability to work independently while collaborating across disciplines
- Strong communication skills and a patient-centered approach to care
- Reliable transportation to travel between facilities
