*Overview* We are seeking a compassionate, knowledgeable and dedicated Hospice Director of Nursing to join our San Antonio team. The ideal candidate will possess a strong background in nursing with experience in hospice management. This role is essential in providing high-quality end-of-life care to patients and support to their families, ensuring comfort and dignity during this critical time. The right candidate will work collaboratively with a multidisciplinary team to develop and implement individualized care plans. MUST HAVE HOSPICE MANAGEMENT EXPERIENCE.

- Provide comprehensive nursing care to hospice patients in accordance with established care plans.
- Conduct assessments of patients’ physical, emotional, and spiritual needs.
- Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care.
- Manage and supervise nursing staff, ensuring adherence to best practices and regulatory standards.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Utilize EMR and EHR systems for documentation review and management of patient information.
- Educate patients and families on disease processes, medications, and end-of-life issues.
- Develop care plans that address the unique needs of each resident while considering their preferences.
- Provide leadership in shift management and oversee the daily operations of the hospice unit.
*Requirements*
- Valid nursing license with experience in hospice care, rules and regulations.
- Knowledge of medicare rules for participation.
- Strong documentation skills with experience using EMR/EHR systems.
- Proven leadership abilities with experience in nurse management or supervising staff.
- Excellent communication skills to effectively interact with patients, families, and healthcare teams.
- Understanding of HIPAA regulations to ensure patient confidentiality is maintained.
