Registered Nurse Liaison

Summary

*

Directly involved in the management of patients serviced by Pharmacare. This management includes clinical assessment of response to therapies, patient education, and home visits to assure Pharmacare therapies are administered correctly. Interfaces with home agencies to ensure continuity of care for clients of Pharmacare infusion therapies.

*Duties Required:*

Assure proper placement of patients of patients within the Home Health Care setting by gathering preadmission information, to assist in better communication of information about patient to all disciplines and oversees the patient’s transition to home care.

Communicates timely with discharge planner/social worker to coordinate needed Home Care services on patients referred to Pharmacare.

Coordinates nursing services relative to delivery of IV therapy to patients at home. This includes initial assessments and reassessments.

Supervises teaching and evaluation of patients prior to discharge and/or at home.

In-services licensed personnel who perform services relative to therapies at home.

Coordinates follow-up visits and calls to patients for inventory, progress, status changes, etc. to assist in coordinating the delivery of supplies and obtain nursing information.

Contributes to the development of professional and patient education materials.

Processes medical information for individual patients.

Performs any other miscellaneous duties as required by Patient Care operations at Pharmacare.

Effectively Communicates with Marketing Staff and Development of Educational and promotional programs

Provides information on patients to the billing and claims departments as needed.

Assist with delivery of drugs and supplies to meet patient requirements, as needed.

*Working Conditions:*

Nursing visits to the patient’s home, hospital, Clinic, MD office or Long Term Care Facilities.

*Minimum

Qualifications

Licensed Registered Nurse in the State of Hawaii.

Associate Degree in Nursing

MUST be able to pass a background check and drug screen

Must be an RN with at least two years experience (within the last three years) that includes administration of IV therapies on a frequent basis.

Experience with skilled nursing in the home setting is preferred.

CRNI certification preferred. Intent to pursue this certification is desirable.

Nursing experience that has included the opportunity to apply principles and practices of intravenous therapy.

Evidence of teaching skills and experience in teaching manual tasks.

Evidence of good communication skills, leadership ability, integrity, initiative, and creativity.

Experience in working with other disciplines in the medical field. (i.e.: Pharmacists, physicians, dieticians, etc.)

In addition to meeting the requirements of the Board of Registered Nurses, stays abreast of developments in the field of intravenous therapy, parenteral nutrition, and practices for the delivery of intravenous therapies to the patient in the home setting.

Job Type: Full-time Pay: From $55.00 per hour
