- Must be licensed in GA & SC
- Respond promptly to after-hours patient calls, providing expert nursing advice and guidance for symptom management and emergency situations
- Conduct thorough patient assessments over the phone or in-home visits to evaluate symptoms, medication needs, and overall condition
- Collaborate with interdisciplinary healthcare teams to coordinate hospice services, including medication adjustments and care planning
- Manage urgent clinical cases by utilizing critical care knowledge, including ICU, emergency medicine, and trauma experience, to stabilize patients as needed
- Document all interactions accurately within electronic health records (EHR), maintaining compliance with HIPAA regulations and medical terminology standards
- Assist with discharge planning and transition of care by communicating effectively with patients, families, and healthcare providers
- Prioritize patient safety by monitoring vital signs, assessing physiological changes, and managing complex symptom control plans
Requirements
- Valid RN license with current registration in the state of Georgia and South Carolina
- Proven clinical experience in critical care settings such as ICU, emergency room, or trauma centers; experience in hospital medicine or post-acute care is highly desirable
- Strong background in medical-surgical nursing, home health care, or hospice services with knowledge of medical terminology and health regulation policies
- Ability to perform comprehensive patient assessments and manage clinical case documentation efficiently using EMR/EHR systems
- Excellent communication skills for client interaction and collaboration with healthcare professionals across disciplines
- Experience with discharge planning, utilization management, diagnostic evaluation, and managing patients as a nurse case manager
- Knowledge of physiology principles relevant to acute care and palliative settings; familiarity with infectious disease care and behavioral health is advantageous
- Flexibility to work on-call hours during evenings, nights, weekends or holidays; ability to respond swiftly in high-pressure situations
