About this role
Remote Registered Nurse position with CareTalk Health involving virtual care, care management, care navigation, and Annual Wellness Visit (AWV) chart preparation. The RN Care Manager manages medically complex patients, develops and monitors care plans across multiple programs (CCM, RPM, RTM, APCM), completes AWV chart preparation, closes care gaps, coordinates orders, and supports preventive and chronic care initiatives. Requires active unrestricted RN license in a compact state, minimum 2-3 years clinical nursing experience, telehealth or virtual care experience, and proficiency with EHR systems. Compensation is $35.00/hour. Contract/1099 position with up to 40 hours per week.
