About this role
We are seeking a compassionate and experienced Hospice RN Case Manager to join our dedicated team. In this role, you will provide comprehensive end-of-life care, coordinate patient-centered plans, and support families during a critical time. Responsibilities include managing a caseload of hospice patients, performing clinical assessments, collaborating with interdisciplinary teams, and ensuring compliance with care standards.

Requirements
- Current Registered Nurse (RN) license in the state of practice
- 1 year of experience as an RN in a hospice or related healthcare setting preferred
- CPR Certification
- BLS Certification
- Valid driver's license and reliable transportation for home visits
- In-home healthcare experience
- Palliative care experience
- Working with geriatric patients
