*What You'll Do*

You'll provide skilled nursing care in the place patients feel most comfortable — their own homes. Your responsibilities include:

  • Delivering care per physician orders, agency policy, and regulatory guidelines
  • Performing OASIS assessments and documentation from admission through discharge
  • Updating and re-evaluating plans of care at least every 60 days
  • Serving as case manager, coordinating multidisciplinary care across nursing, therapy, home health aides, and social services
  • Maintaining accurate, timely clinical documentation
  • Participating in Quality Assurance and Performance Improvement (QAPI) programs
  • Ensuring all care meets compliance and regulatory standards

*Who We're Looking For*

  • Active NYS Registered Nurse license (required)
  • Valid NY Driver's License, CPR/BLS certification, and COVID-19 vaccination (as applicable)
  • 1+ year of RN experience preferred — new graduates are welcome and will receive full onboarding support
  • Home care, acute care, or rehab background is a plus
  • OASIS experience helpful but not required
