What You’ll Do as an RN Case Manager
Complete home health intake assessments, medication reconciliation, and care plans
Collaborate with physicians, caregivers, and interdisciplinary teams
Educate patients, families, and caregivers on disease management, safety, and recovery
Maintain accurate clinical documentation and communication with staff
Qualifications
Current RN license in Washington State
Minimum 2 years nursing experience; home health experience preferred but not required
CPR certification
Licensed driver with reliable, insured vehicle
Strong communication, clinical judgment, and patient-focused care skills
