WE'RE HIRING! Registered Nurse (RN) – Structured Family Caregiving & Wellness Manager
Helping Hands In-Home Care, Inc. is looking for an experienced, organized, and compassionate Registered Nurse (RN) to join our team and oversee our Structured Family Caregiving (SFC) and Wellness programs .
This is an excellent opportunity for an RN who enjoys case management, client relationships, caregiver education, community-based nursing, and working independently rather than providing traditional bedside nursing.
The RN – Structured Family Caregiving & Wellness Manager will manage an assigned caseload of individuals receiving Structured Family Caregiving and Wellness services . This position will provide nursing oversight, complete required assessments and visits, monitor client well-being, support family caregivers, and coordinate with case managers, guardians, healthcare providers, and the Helping Hands team.

What You'll Do
- Manage Structured Family Caregiving (SFC) and Wellness cases .
- Complete required nursing assessments, reassessments, and client visits.
- Monitor clients for changes in condition, health and safety concerns, and changes in support needs.
- Provide ongoing education and support to Structured Family Caregivers.
- Serve as a nursing resource for clients, caregivers, families, and agency staff.
- Coordinate with case managers, guardians, physicians, hospitals, pharmacies, and other providers as appropriate.
- Review hospital discharge information, medication changes, physician recommendations, and other relevant healthcare documentation.
- Provide wellness education and preventative support based on each individual's needs.
- Assist clients and caregivers with understanding health-related recommendations and when additional medical attention may be necessary.
- Identify concerns involving falls, skin integrity, nutrition, hydration, infection prevention, medication management, caregiver burnout, and other potential risks.
- Participate in person-centered planning and interdisciplinary meetings as needed.
- Complete required documentation accurately and on time.
- Ensure services and documentation remain consistent with each client's authorized service plan.
- Assist with incident follow-up, quality assurance, and compliance reviews.
- Maintain communication with agency management regarding significant client or caregiver concerns.
- Help ensure client records remain complete, accurate, and survey-ready .
Required
- Current, unrestricted Indiana Registered Nurse (RN) license .
- Graduate of an accredited nursing program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and ability to travel to client homes.
- Strong assessment and clinical judgment skills.
- Excellent communication and documentation skills.
- Ability to independently organize and manage a caseload.
- Comfortable working with clients, families, guardians, caregivers, and interdisciplinary teams.
- Ability to use electronic documentation and client-management systems.
Preferred
- Home care, home health, community nursing, case management, developmental disability, or Medicaid waiver experience.
- Experience working with individuals with intellectual and developmental disabilities.
- Knowledge of Indiana HCBS waiver services.
- Experience with Structured Family Caregiving.
- Experience educating and supporting family caregivers.
Enjoys nursing but is looking for something different from traditional bedside care. This role focuses heavily on relationships, prevention, education, oversight, case management, and helping individuals successfully remain in their homes and communities.
