Serves a home-based patient population requiring nursing, rehabilitation, hospice, and personal care services

16-week assignment
Requirements
Active RN license in VT and NH or unencumbered compact license
BLS/BCLS certification required
Graduate of an accredited school of nursing
Minimum 2 years of experience in home health

Additional Information
Develop and periodically review individualized care plans in collaboration with physicians, patients, and families
Coordinate care through interdisciplinary communication and case conferences, including timely discharge notifications
Provide ongoing clinical, functional, and emotional assessments and communicate significant changes to physicians and colleagues
Document evaluations, treatment goals, and objectives
Teach patients and caregivers self-care techniques and provide medication, diet, and health counseling as ordered
Work independently while remaining part of a cohesive interdisciplinary care team
