How you’ll make an impact in this role

Coordinate interdisciplinary rehabilitation care in close partnership with PT, OT, and SLP specialists to ensure unified, progress-driven treatment plans. Monitor and communicate critical health changes in real time, delivering accurate clinical assessments to physicians to maintain safety during intensive therapy. Execute direct nursing interventions and physician orders with precision, supporting physical recovery and helping patients rebuild daily functional independence. Drive comprehensive discharge planning alongside the care team, empowering patients and families with the tools and education needed for a safe transition home.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

One or more of the following required

Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date. Education

Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
