We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care. Responsibilities include reviewing patient information, supervising and mentoring clinical staff, conducting performance evaluations, facilitating clinical training, collaborating with interdisciplinary teams, monitoring patient outcomes, assisting in screening and interviewing new personnel, and performing other duties as delegated by the Clinical Director.
At least one year of management or supervisory experience in the home health setting
Strong leadership and interpersonal skills
Familiarity with OASIS, care documentation requirements, and interdisciplinary coordination
Experience using electronic medical record (EMR) systems such as Kinnser
Knowledge of clinical best practices and regulatory standards
Commitment to maintaining patient confidentiality and ethical standards
