*Position Overview*
We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the comfort of patients’ homes. The ideal candidate is clinically strong, independent, and committed to improving patient outcomes through personalized care and education.
*Key Responsibilities*
- Perform intake for admission, discharge, transfer, comprehensive patient assessments, including physical, emotional, and environmental needs
- Develop, implement, and update individualized care plans
- Administer medications, treatments, and wound care as prescribed
- Monitor patient progress and document clinical findings accurately

- Educate patients and families on disease management, medications, and self-care
- Collaborate with physicians, therapists, and interdisciplinary care teams
- Ensure compliance with all healthcare regulations and agency policies
- Respond to changes in patient condition and provide appropriate interventions
*Qualifications*
- Active Registered Nurse (RN) license in Alaska
- Minimum of1 year of nursing experience is preferred(home health experience preferred)
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
- Current CPR/BLS certification
*Preferred Qualifications*
- Experience with electronic health records (EHR)
- Knowledge of Medicare/Medicaid guidelines for home health
- Wound care certification or experience
