Registered Nurse (RN) – Floating Visit Nurse | Home Health. As a Floating Visit Nurse, you'll experience the full scope of home health nursing by working with different patient populations, supporting multiple clinical teams, and caring for patients throughout the Twin Cities Metro. Full-time position with consistent patient volume. Work across multiple Twin Cities territories. Provide skilled home health nursing visits throughout the Twin Cities service area. Complete comprehensive patient assessments. Perform wound care, medication management, injections, and other skilled nursing procedures. Educate patients and caregivers on treatment plans and disease management. Coordinate care with physicians and interdisciplinary team members. Provide coverage across territories to support patient needs and growing census. Complete accurate documentation using Homecare Homebase (HCHB).

Requirements
- Active Minnesota Registered Nurse (RN) license
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, hospital, rehabilitation, skilled nursing, or acute care preferred
- Strong assessment and critical thinking skills
- Comfortable adapting to different clinical environments
- Excellent communication and time management abilities
- Reliable transportation with the flexibility to travel throughout the Twin Cities Metro
- Homecare Homebase (HCHB) documentation experience
- Skilled nursing facility experience
