About this role

Are you an experienced Registered Nurse with a background in home health who enjoys working independently and making a meaningful impact in the community? This long-term opportunity as a Home Health Nurse Assessor allows you to conduct in-home assessments that help ensure patients receive the care and services they need.

If you are passionate about community-based care, geriatric health, and comprehensive patient assessments, this role offers a rewarding career path with long-term stability.

This is a Hybrid set-up, only LOCAL candidates may apply.

Rate increase valid for caregivers submitted during May and starts in June

The schedule is flexible that you can start earlier in the day and also get 3 assessments per day versus 2 visits. You can also work on weekends.

Minimum Requirements

Candidates must have at least 2 years of home-based healthcare experience within the last 10 years, including one or more of the following:

Field work with a Certified Home Health Agency (CHHA)

Work with a Licensed Home Care Services Agency (LHCSA)
