Responsibilities

Registered Nurse (RN)

As a Registered Nurse, you will provide skilled nursing care in a collaborative outpatient environment while helping patients achieve optimal health outcomes.

Key Responsibilities

Deliver skilled nursing services under the direction of physicians and advanced practice providers Assess patient health needs and analyze clinical data Develop, implement, and evaluate individualized plans of care Monitor patient progress and modify care plans as appropriate Collaborate with providers and interdisciplinary healthcare teams Educate patients and families regarding health conditions, treatments, and preventive care Maintain a safe, therapeutic, and patient-focused environment

Registered Nurse Care Coordinator (RNCC)

As a Registered Nurse Care Coordinator, you will play a critical role in supporting patients across the continuum of care, with a focus on coordination, communication, and improving health outcomes.

Key Responsibilities

Partner with providers and clinic teams to deliver evidence-based, patient-centered care Support patients in managing care plans and achieving health goals Coordinate care for high-risk and medically complex patients Assist patients with chronic conditions, disabilities, and disease-specific care management needs Facilitate smooth care transitions, including hospital discharge follow-up and transitions between care settings Serve as a liaison among patients, families, healthcare providers, and community resources Promote comprehensive, whole-person care through proactive communication and care coordination

Qualifications

Minimum Qualifications Required for Both Positions

Associate Degree in Nursing from a CCNE or ACEN accredited nursing program Current Ohio Registered Nurse license Current CPR/BLS certification Experience in clinic, physician office, urgent care, or related healthcare settings Knowledge of triage procedures and patient care standards Strong communication and patient education skills Ability to maintain quality, safety, and infection control standards Excellent interpersonal, organizational, and critical-thinking skills Ability to maintain confidentiality and handle sensitive situations with professionalism and diplomacy
