About this role
The Enrollment Nurse is responsible for completing the UAS assessment, reviewing the NYIA assessment, creation of the time tasking tool for the potential member, and development of the Person-Centered Service Plan (PCSP). The Enrollment nurse also conducts PCSP meeting with the potential member and obtains the enrollment documentation to ensure potential member enrolls into the Plan.
