About this role

Creekside Post Acute, a leading provider of short-term and long-term skilled rehab services, is seeking a compassionate and skilled Registered Nurse (RN) Wound Care Nurse to join our team in Hopewell, VA. As a Wound Care Nurse, you will play a vital role in providing high-quality care to our residents, focusing on wound prevention, wound care, ostomy care, and incontinence management.

Responsibilities

  • Develop and implement policies and procedures to manage residents with draining wounds, fistulae, and/or tubes
  • Provide education to residents undergoing surgery resulting in an ostomy, including pre- and post-operative assessments and resident and family education
  • Make written or oral reports concerning the activities of your shift, as required
  • Ensure all nursing service personnel comply with procedures set forth in the Nursing Service Procedures Manual

As a Registered Nurse (RN) Wound Care Nurse, you will have the opportunity to integrate research findings into clinical practice, demonstrating knowledge and skills to develop and implement effective care plans.
