Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.

You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.

What You'll Do

  • Provide direct patient care in accordance with the state Nurse Practice Act
  • Implement and support the plan of care developed by the RN
  • Deliver skilled nursing services, including treatments, medication support, and wound care
  • Monitor patient condition and report changes to the RN and care team
  • Educate patients and caregivers on self-care, treatments, and disease management
  • Maintain infection control and proper use of equipment and supplies
  • Document patient care accurately and timely within the EMR
  • Communicate effectively with physicians, nurses, and interdisciplinary team members
  • Participate in case conferences, team meetings, and ongoing training
  • Support coordinated care by maintaining consistent communication across the care team

What You Bring

Active LPN/LVN license in the state of practice Graduate of an accredited practical/vocational nursing program Current CPR certification Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus) Strong clinical and observational skills Clear communication and patient education abilities Ability to follow care plans while recognizing and reporting changes in condition Basic proficiency with EMR systems Valid driver's license and reliable transportation
