RN Case Manager Home Health

Quality Life Services is a growing home health agency based out of Fairfield, Ohio. We are currently looking for a passionate Registered Nurse Case Manager in Hamilton County. As Case Manager you would be responsible for to oversee your patients individualized plan of care from admission to discharge with assistance from PT/OT/ST and MSW as needed.

Responsibilities and Duties

• Assess the patient’s physical and functional needs

• Perform a home safety assessment

• Conduct a complete medication review

• Work with patients and families to best meet the patient’s needs, as well as the needs of the people that support them at home

• Establish a personalized plan of care

• Monitor and supervise the care provided by HHA or LPN

Qualifications and Skills

• Registered nurse license in the state of Ohio

• Home Care experience is preferred, but not required

• Minimum 1 year experience as a Registered Nurse

• Reliable transportation with proof of insurance'
