Assess & Understand: Conduct initial and ongoing bio-psychosocial assessments using trusted nursing standards, creating a personalized care plan that aligns seamlessly with each patient's master treatment plan. Collaborate & Connect: Join supportive treatment team conferences to help set, adapt, and refine goals, objectives, and care strategies tailored to each patient's unique background and needs. Support & Facilitate: Bring nursing care plans to life for your assigned patients, and lead or co-lead engaging group therapy sessions that foster healing and growth.

Evaluate & Evolve: Walk alongside patients to evaluate how they respond to care, teaming up with colleagues to update plans and interventions whenever a patient's journey takes a new step. Licensure / Certification / Registration Required Credential(s): BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

Registered Nurse obtained prior to hire date or job transfer date. Education Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
