About this role
The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of he community to plan and direct services to meet the needs of individual and families within their homes and communities.
- Conducts comprehensive and ongoing assessments of both the patient and family to determine home health and hospice needs, including physical, psychosocial, and spiritual status.
- Utilizes health data to establish nursing diagnoses and sets measurable goals that incorporate palliative actions.
- Writes, initiates, and regularly revises the POC based on evolving patient status and caregiver needs.
- Executes preventive and rehabilitative procedures, administers prescribed medications, and helps patients with activities of daily living to maximize comfort.
- Instructs patients and families on self-care techniques, medication, and diet while providing ongoing health and coping counseling.
- Maintains and submits accurate notes regarding patient conditions, care administered, and pain/symptom management outcomes.
- Reports clinical changes and needs to the attending physician to secure required orders.
- Acts as Case Manager to coordinate the care plan with other health professionals and caregiver.
