Health Home Registered Nurse Case Manager

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while managing a diverse patient caseload.

Responsibilities

• Conduct comprehensive patient assessments, including vital signs and health history reviews.

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.

• Administer medications and treatments as prescribed, including IV infusion and tube feeding.

• Monitor patient progress and adjust care plans as necessary based on clinical evaluations.

• Provide education to patients and families regarding health management, medication administration, and lifestyle modifications.

• Document all patient interactions accurately in electronic health records.

• Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes.

• Participate in ongoing training and professional development to enhance nursing skills.

Experience

• Current RN license in the state of practice.

• Strong understanding of vital signs monitoring and patient assessment protocols.
