*Caring Hearts Hospice* is seeking a compassionate, dedicated, and experienced *Registered Nurse* to join our hospice team in a full-time regional coverage role.
This position is ideal for an experienced hospice nurse who enjoys variety, and supporting patients and team members throughout our service territory. The Hospice RN provides skilled nursing visits, assessments, symptom management, patient and caregiver education, and compassionate end-of-life care wherever our patients call home.

Hospice Philosophy
Hospice care is an interdisciplinary approach designed to meet the physical, psychological, emotional, and spiritual needs of individuals at the end of life, with a focus on comfort rather than cure.
Hospice recognizes that the unit of care extends beyond the patient to include the family and caregivers as defined by the patient.
Responsibilities
The Hospice RN provides compassionate, skilled nursing care to patients throughout the Caring Hearts Hospice service territory.
Responsibilities include
- Providing scheduled hospice nursing visits and coverage based on weekly patient and staffing needs
- Traveling throughout the service territory, with the assigned coverage area potentially changing from week to week
- Completing comprehensive nursing assessments and identifying changes in patient condition
- Providing symptom management and comfort-focused nursing interventions
- Educating and supporting patients, families, and caregivers
- Collaborating with physicians, hospice leadership, and the interdisciplinary care team
- Completing timely and accurate clinical documentation
- Communicating changes in patient condition and coordinating appropriate follow-up
- Providing care wherever the patient calls home, including private residences, assisted living communities, adult foster homes, skilled nursing facilities, and other settings
- Supporting continuity of care for patients when their regularly assigned case manager is unavailable or additional nursing coverage is needed
*Daily driving and travel throughout our service territory are essential functions of this position.*
Qualifications
- Graduate of an accredited School of Nursing
- Current Registered Nurse license in the State of Oregon
- Minimum of one year of RN experience
- *Hospice nursing experience required*
- Strong clinical assessment and critical-thinking skills
- Excellent communication, organization, and problem-solving abilities
- Ability to work independently and adapt to changing weekly assignments
- Comfortable traveling throughout the Caring Hearts Hospice service territory
- Valid driver's license and access to an insured, reliable automobile in good working order
- BLS certification preferred
