This a culture of winning. At TMS we breed winners. Responsibilities Provide general licensed practical nursing care to home-bound patients with accurate documentation. Assess patient needs and update care plans based on changes in condition. Administer medications and monitor for adverse reactions per protocols. Document all visits, interventions, and patient progress in electronic health records. Coordinate with physicians and family/c caregivers to ensure continuity of care. Use data and KPIs to track patient outcomes and contribute to care quality. Provide education to patients and families on disease management and safety at home. Meet productivity targets and complete visits within scheduled timeframes. Deliver compassionate, patient-centered care while adhering to privacy and confidentiality standards. Identify and report safety concerns, equipment needs, and supply shortages. Maintain professional licensure, attend mandatory trainings, and pursue development opportunities. Experience/Qualifications Current LVN/LPN license in good standing with state board Proven home health nursing experience with patient assessments Medication administration and IV/technique competence per protocols Accurate documentation in electronic health records and care plans Commitment to privacy, confidentiality, and HIPAA compliance Strong communication with physicians, patients, and caregivers Ability to meet productivity targets and complete visits on schedule Data-driven mindset; track KPIs and contribute to quality metrics Patient education on disease management and home safety On-site visits; reliable transportation and valid driver’s license What's in it for you?
