About this role
Guidehealth is seeking a dedicated and compassionate Registered Nurse (RN) to join their team as a Telehealth Medicare Annual Wellness Visit Specialist - PRN. In this remote role, the RN will conduct Medicare Annual Wellness Visits (AWVs) via telehealth under the direct supervision of a physician. Responsibilities include conducting AWVs per CMS guidelines, reviewing and updating patient medical histories, assessing patients' current health status, providing patient education on preventive measures, developing personalized prevention plans, collaborating with supervising physicians, identifying HCC codes, maintaining HIPAA-compliant patient records, facilitating virtual referrals, and monitoring patient progress.
Requirements include being fully bilingual in Vietnamese and English, located in GA, current unrestricted RN license in GA, PRN availability Monday-Friday 8am-5pm approximately 20 hours per week, minimum 3 years patient care nursing experience preferably in primary care/preventive care/telehealth, experience with Medicare AWVs and telehealth platforms, proficiency in EHR systems and telehealth technology.
