We are seeking a Telemetry LPN to provide direct patient care in a fast-paced telemetry/acute care unit. This role includes cardiac monitoring, assessment of vital signs, medication administration, and documentation. The Telemetry LPN monitors patient conditions, reports changes promptly, and collaborates with RNs and physicians to ensure safe, high-quality patient care.

Responsibilities

Essential Duties and Responsibilities (include, but are not limited to)

Monitor cardiac rhythms and vital signs using telemetry equipment, identifying and reporting abnormalities Assess, record, and document vital signs and patient status accurately in the medical record Notify clinical staff of significant changes in patient condition or telemetry readings Assist patients with activities of daily living (ADLs) and provide basic care support Educate patients and families on telemetry monitoring and collaborate with RNs to support care delivery

  • Current and valid state license
  • Current BLS certificate upon hire and maintain current
  • IV certification upon hire (state specific)
