- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine appropriate care strategies.
- Develop and implement individualized care plans that align with clinical best practices, hospital policies, and patient needs.
- Manage inpatient and outpatient cases across diverse settings such as hospital medicine, emergency rooms, intensive care units (ICU), pediatric units, and post-acute care facilities.
- Collaborate closely with healthcare professionals—including physicians, therapists, social workers, and discharge planners—to coordinate services and optimize patient outcomes.
- Oversee utilization management by reviewing diagnostic evaluations, managing hospital resources efficiently, and ensuring compliance with health regulation policies like HIPAA.
- Facilitate discharge planning by coordinating home health services, hospice care, or outpatient follow-up to support smooth transitions from hospital to home or other care facilities.
- Maintain accurate medical records using Electronic Health Records (EHR) systems while ensuring confidentiality and adherence to HIPAA regulations.
*Skills*
- Previous Home-Health experience required
- Proven experience in critical care environments such as Level I Trauma Centers, Level II trauma centers, or emergency medicine settings.
- Strong background in hospital medicine, medical-surgical nursing, ICU care, or behavioral health with a focus on patient service excellence.
- In-depth knowledge of physiology, medical terminology, infectious disease care, and post-acute care processes.
- Skilled in patient assessment techniques including vital signs monitoring and diagnostic evaluation.
- Proficiency in EMR/EHR management systems along with understanding of health regulation policies like HIPAA.
- Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across inpatient and outpatient settings.
- Experience managing diverse patient populations including geriatrics, pediatrics (PICU), or patients requiring hospice or home health services.
- Ability to navigate managed care protocols and utilization management procedures efficiently while maintaining a compassionate approach to patient care.
