In this rewarding role, you will assess the health-related needs of our clients by monitoring ongoing health as well as provide health services for clients.

What You’ll Do
- Conduct assessments of substance users and family members to determine appropriateness for participation in treatment, need for physical exam or other medical services.
- Follow up on any health-related referrals made
- Maintain a tracking system to monitor patient health issues, medical appointments, and prescribed medications (internal/external)
- Provide patient and staff education around health-related issues.
- Provide crisis intervention on an emergency basis when indicated.
- Complete documentation and reports in a timely manner.
- Participate in staff meetings, case conferences and other unit meetings.
- Maintain communication and teamwork with clinical staff to integrate and enhance clinical services.
We require
- Valid NYS RN Licensure
- Experience working as a Registered Nurse in a substance use disorder treatment facility setting preferred
- Compassionate, organized leader with excellent communication skills
