About this role

We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team to provide home health care to individuals with disabilities. Candidate will provide skilled nursing care, medication management, and other services tailored to the clients needs. RNs in this setting work to support independence, maintain health, and promote overall well-being in the home environment.

As an RN, you will play a crucial role in supporting clients health and well-being, assisting with daily living activities, and ensuring a safe and nurturing environment for all patients.

Duties

  • Administer medications and treatments as prescribed by physicians.
  • Provide catheter care and assist with other personal hygiene needs.
  • Monitor patients' vital signs and report any changes to the healthcare team.
  • Assist in the development of patient care plans in collaboration with care team
  • Support patients with aliment-related issues, ensuring their comfort and understanding of their health conditions.
  • Collaborate with multidisciplinary team to provide comprehensive patient care.

Qualifications

  • Valid RN license in the state of Pennsylvania to practice.
  • Previous experience in home and community care settings preferred.
  • Strong communication skills and the ability to work effectively within a team environment.
  • Compassionate demeanor with a commitment to providing high-quality patient care.
  • Ability to safely assist clients with mobility as needed.
