  • Coordinate and manage in-home supportive services for clients, including assessing individual needs and developing tailored care plans.
  • Collaborate closely with healthcare professionals, social workers, and family members to ensure comprehensive patient care.
  • Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform care strategies.
  • Oversee discharge planning and post-acute care coordination to facilitate smooth transitions from hospital to home or other settings.
  • Maintain accurate and up-to-date medical records using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA regulations.
  • Manage case documentation related to medical records, service utilization, and clinical case management activities.
  • Provide patient service by addressing concerns promptly, educating clients on health management, and supporting behavioral health needs when applicable.

*Skills*

  • Extensive critical care experience with a strong background in hospital medicine, emergency medicine, or intensive care units (ICU).
  • Proven experience working within Level I Trauma Centers or Level II trauma centers; familiarity with trauma protocols is highly desirable.
  • Knowledge of infectious disease care, geriatrics, behavioral health, and post-acute or hospice settings enhances your ability to deliver comprehensive support.
  • Skilled in managing patients as a nurse case manager with expertise in discharge planning, utilization management, diagnostic evaluation, and medical-surgical nursing.
  • Proficient in EMR/EHR systems for electronic health records management; understanding of HIPAA policies ensures confidentiality.
  • Strong communication skills for client interaction and collaboration with multidisciplinary healthcare teams.
  • Ability to perform patient assessments accurately by monitoring vital signs and conducting clinical evaluations; familiarity with physiology is essential.
  • Experience in managing home health services, outpatient clinics, or inpatient settings; knowledge of medical terminology is required.
  • Demonstrated understanding of health regulation policies related to managed care and patient safety standards.
