The Community Living Center (CLC) Resident Assessment Coordinator (RAC) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. The nurse leads the CLC Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) program, serves as Facility Administrator of the RAI/MDS program and MDS software, directs the RAI/MDS RN, conducts interrater reliability reviews quarterly, and collaborates in development, implementation, evaluation, and revision of policies, procedures, and guidelines. The nurse is an active member of an interdisciplinary team focused on care transitions to and from an inpatient setting.

Requirements

  • U.S. Citizenship
  • Current, full, active, and unrestricted RN license in a State, Territory, or Commonwealth of the United States, or the District of Columbia
  • Graduate of a school of professional nursing accredited by ACEN or CCNE
  • English Language Proficiency (basic written and spoken English)
  • Background/security investigation
  • Drug screening (urinalysis)
  • Pre-employment physical examination
  • Selective Service Registration required for males born after 12/31/1959
