Registered Nurse (RN) - Patient Navigator

Role Overview: Provides patient referrals, education, and guidance for orthopedic and cardiac DRGs, ensuring care aligns with evidence-based and regulatory standards. Responsibilities: Coordinates referrals from physicians and healthcare facilities, delivers comprehensive patient education about illnesses and treatments, and guides patients through the care continuum. Completes required competencies and performs related duties as assigned. Qualifications & Skills Associates Degree in Nursing (Bachelors preferred) Minimum of 3 years clinical experience in the specialty area Current Texas RN license; specialty certification preferred Active healthcare provider BLS certification (AHA or ARC), with AHA Healthcare Provider BLS within 60 days Key Attributes: Strong patient education skills, knowledge of evidence-based standards, regulatory compliance, and team collaboration.
