Responsibilities

Provide direct patient care as part of a multidisciplinary team, including administering medications, performing vital signs, and conducting treatments

Develop and implement care plans tailored to individual patients' needs, taking into account their physical, emotional, and social well-being

Collaborate with other healthcare professionals to ensure seamless communication and coordination of patient care

Participate in shift bed counts, patient flow, and other administrative tasks as needed

Demonstrate a commitment to patient-centered care and a strong understanding of the importance of compassion, empathy, and respect in the delivery of patient care

Requirements

Current LPN licensure in the State of Pennsylvania

High school diploma or equivalent required

Previous experience in a long-term care or subacute setting preferred
