About this role

The home health registered nurse is responsible for providing skilled nursing care to patients in their homes, following a physician-approved plan of care. The nurse assesses patient needs, develops and implements care plans, educates patients and caregivers, and coordinates care with an interdisciplinary team to promote patient independence and optimal health outcomes. Responsibilities

Patient Assessment & Care: Conduct initial and ongoing comprehensive assessments of patients in their home environment. Develop, implement and evaluate individualized plans of care. Perform skilled nursing procedures, including wound care, urinary catheters, medication management, injections, and ostomy care. Documentation & Compliance: Maintain accurate, timely and complete electronic medical records (EMR) in compliance with agency policies, state regulations, and Medicare guidelines (including OASIS documentation). Patient & Family

Education

Educate patients and their caregivers on disease processes, medication administration, safety, and self-care techniques to empower them to manage their own health. Interdisciplinary Collaboration: Communicate and collaborate effectively with physicians, therapists, and social workers to ensure coordinated and holistic patient care. Safety & Advocacy: Advocate for patient needs and ensure a safe home environment. Recognize and report any changes in a patient's condition or safety concerns.

Requirements

  • HOME HEALTH: 1 year
  • Graduate of an accredited school of nursing
  • Current, unrestricted RN license in the state of practice
  • Minimum of one year of clinical experience
