LPN/LVN On-Call - Part Time

LVN/LPN On Call - Part Time Saturday & Sunday 8AM - 8PM What You Must Have • Graduate of an accredited school of vocational nursing • Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse • Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting, hospice or home health preferred • Current State Driver's License • Must have reliable transportation and provide ongoing valid and current auto liability insurance Full-Time Benefits • Competitive Pay • Medical, Dental & Vision insurance • Paid Time Off • Paid holidays • 401k with up to 4% employer matching • Tuition reimbursement • Company car for qualifying individuals • Mileage reimbursement What You Will Do • Under the direction of the On Call RN/Triage RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care (POC) • Provide home visits as directed by the On Call RN/Triage RN to observe and report findings to the appropriate RN. • Assist the RN in caring out the POC • Assist the patient in learning appropriate self-care techniques • Provide services in accordance with organizational policies and procedures and regulations that define the scope of services • Assist the RN and/or physician in performing specialized duties related to end-of-life care • Timely submission of all documentation • Performance of other duties as required • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures It is this Agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
