- Conduct thorough home assessments to evaluate patient health status, functional abilities, and environmental safety.
- Perform quarterly reviews to monitor patient progress, update care plans, and adjust interventions as needed.

- Manage patient case documentation accurately within electronic health records (EHR) systems, ensuring compliance with HIPAA and health regulation policies.
- Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to coordinate personalized care strategies.
- Provide patient education on medication management, disease processes, and self-care techniques tailored to individual needs.
- Facilitate discharge planning and transition of care from hospital or facility to home setting, including post-acute care coordination.
- Utilize clinical judgment for diagnostic evaluation and medical record review to inform ongoing treatment plans.
Experience
- Proven experience as a Registered Nurse (RN), with a strong background in home health or outpatient settings.
- Critical care experience preferred, including ICU, emergency medicine, or medical-surgical environments.
- Experience working within Level I or Level II trauma centers is highly desirable.
- Knowledge of infectious disease care, geriatrics, behavioral health, and hospice is advantageous.
- Familiarity with electronic health records (EHR/EHR management), medical terminology, and healthcare compliance standards such as HIPAA.
- Demonstrated ability to perform comprehensive patient assessments and manage complex cases effectively.
- Strong communication skills for client interaction and interdisciplinary collaboration are essential.
