About this role

Transitions At Home, is currently seeking to hire a Full-Time compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients in the Mount Horeb area.

What does a Transitions at Home LPN do? Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

  • Hours: Full-Time
  • 40 hours per week 8:00 am - 4:30pm
  • Monday through Friday
  • No Night Shifts or Weekends
  • Limited on-call rotation
