- *Shift*: 7:00 PM to 7:00 AM & 03:00 PM to 11:00 PM & 07:00 AM to 3:30 PM
- *Schedule*: 4-5 days/week (facility schedule)
- *Guaranteed Hours*: 36 to 40 hours/week
- *Pay Rate*: $40-49/hr
*Roles & Responsibilities*
- Provide direct nursing care according to individual patient/resident care plans
- Administer medications and treatments safely and accurately
- Monitor, assess, and document patient/resident conditions
- Assist with wound care, treatments, and coordination of nursing tasks
- Communicate effectively with RNs, physicians, families, and facility staff
- Maintain accurate documentation and follow facility, state, and federal guidelines
- Promote a safe and high-quality care environment
*Requirements*
- Active LPN license in New Hampshire/Compact License
- Minimum 1 year of LPN experience (LTC/SNF preferred)
- Must be reliable, professional, and able to work consistently
- Strong clinical judgment and communication skills
