About this role

Coordinate admissions for residents with challenging behaviors and financial/discharge planning risks. Collaborate with the interdisciplinary (ID) team to ensure smooth resident admissions and transitions. Assist with User-Defined Assessments (UDAs).

Enter physician orders and discontinue completed orders as appropriate. Initiate and maintain individualized resident care plans. Accurately document and update resident information in PointClickCare.

Ensure compliance with facility policies, procedures, and regulatory requirements. Requires Registered Nurse License and a Bachelor's degree from an accredited university or college. Previous experience in long-term care and community-based care strongly preferred.

Proficiency with PointClickCare, including order entry and care plan management required. Full-time; Direct Hire. Monday - Friday, 8:00am to 5:00pm.

  • Proficiency with PointClickCare, including order entry and care plan management. Excellent organizational, communication, and critical thinking skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Must be able to read, write, speak, and understand English to communicate effectively. Demonstrates professionalism, flexibility, organization, assertiveness, and cooperation.
