Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life. Key responsibilities include performing comprehensive health assessments, diagnosing and managing acute and chronic conditions, developing evidence-based care plans, providing ongoing medical management, supporting value-based care initiatives, collaborating with physicians and interdisciplinary team members, facilitating transitions of care, educating residents and families, and maintaining accurate documentation.
Requirements include a Master's degree in Nursing from an accredited Nurse Practitioner program, active and unrestricted Advanced Practice Nurse (APN) license, current DEA registration and prescriptive authority, board certification as a Nurse Practitioner (e.g., FNP, AGNP), and 2 years of clinical experience preferred in geriatrics, long-term care, or post-acute settings.
