Constellation Home Care is looking for a compassionate and skilled Registered Nurse (RN) Case Manager to join our team! This is a field-based RN position requiring someone to drive and cover an assigned territory treating patients in their homes, assisted living facilities, and in skilled nursing facilities.

Responsibilities

  • Provide skilled nursing and head-to-toe assessments to the adult and geriatric population. This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.
  • Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.
  • Develop the Plan of Care in coordination with all health care professionals assigned to the case.
  • Be knowledgeable in the use of the current OASIS assessment for all patient admissions, re-certification, transfers and discharges.
  • Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
  • Report to the Clinical Manager on a daily basis.
  • Provide documentation using the provided iPad within the required time frame.

Qualifications

  • Registered Nurse Licensed in the state of employment, graduate of an accredited RN program.
  • At least 1-2 years of RN experience is a MUST ; Home care preferred
  • Oasis experience is strongly desired
  • Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
  • Have reliable transportation and be willing to commute to provide the best in-home patient care.
  • Some weekend days are required, and summer and winter holiday commitments as needed.
  • Basic knowledge of iOs operating system
