The LVN Triage is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Professional Duties • Provides direct patient care as defined in State Nurse Practice Act. • Implements current nursing practice following a patient assessment and an approved plan of care initiated by the registered nurse, as required. • Responds to urgent and family/patients calls, as required. • Provide accurate and timely documentation of patient services to reflect the plan of care. • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care. • Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian or Clinical Supervisor. • Uses equipment and supplies effectively and efficiently. • Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications. • Participates in personal, professional growth and development. Also participates in organization's quality management program. • Performs other duties as assigned. Qualifications • Licensed Vocational Nurse in the State of California. • One year experience as an LVN. • Knowledge of the principles and practices of primary medical and nursing care. Understands the philosophy of hospice concept preferred. • Demonstrates the ability to use good verbal and written communication skills. • Good organizational skills. • Ability to work as a team member. • BLS required within 60 days of hire. • Current California driver's license and proof of insurance. • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position. Physical Demands • Sitting: Approximately 60% of day. • Standing: Approximately > 20% of day. • Walking: Approximately > 20% of day. • Lifting: 0 - 40 lbs. (equipment, supplies) approximately >20% of day. • Bending: Approximately >10% of day. Kneeling < 20%. • Hearing/Visual Acuity: Adequate for use with computers, telephone and/or cellphone. Approximately 50% of day. • Computer: Highly technical work environment. Must be able to work minimum of 4 hours a day using keyboard, mouse and monitor. • Reaching: Above head 75 degrees approximately 25% of day. • Hand grip dexterity: Approximately 40% of day.
