HealthPartners is looking to hire an LPN to join our Palliative Care team! Join us as a Partner for Good and help make an impact on the care and experience that our patients and their families receive every day.
Position Summary
The Palliative Care Licensed Practical Nurse provides nursing care, based on standards of practice, to patients and families in their home or community. This role supports clinicians, registered nurses, social workers and chaplains with patient care in the ambulatory Palliative Care setting. This remote role is telephonic, requiring a high level of professionalism, verbal and written communication skills and strong ability to initiate and follow through with time-sensitive tasks.
Work Schedule
M-F 8am-5pm
Education, Experience or Equivalent Combination
Graduation from a state approved school of practical nursing Licensure/ Registration/
Certification
Current state Licensed Professional Nursing License in good standing Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. Current MN driver’s license
Knowledge, Skills, and Abilities
Ability to exercise tact, initiative, clear communication, and good judgment in dealing with people. Ability to accept and utilize supervision. Ability to work within the team concept
Preferred Qualifications
Work experience as a licensed practical nurse for at least two years. Hospice / Home Care / Palliative Care experience. EMR / Epic experience
