About this role
Hospice Admissions Nurse responsible for executing the hospice admission process in compliance with federal and state regulations. Advocates for patients and families to meet their needs that hospice can cover and refers them to other resources as appropriate. Responsibilities include demonstrating competency with potential patients, hospice eligibility, maintaining confidentiality of clinical records, completing admissions and patient care orders, and communicating effectively with physicians, referral sources, patients, families, and hospice interdisciplinary teams.
