Key Responsibilities
Provide safe, compassionate, and patient-centered nursing care.
Administer medications and treatments according to established orders and protocols.
Monitor patient conditions and promptly report changes to the appropriate healthcare provider.
Perform assessments and document patient care accurately.
Collaborate with RNs, physicians, therapists, aides, and other members of the healthcare team.
Support patients with activities of daily living as appropriate.
Maintain accurate and timely clinical documentation.
Promote patient safety, comfort, dignity, and independence.
Follow all facility, state, and professional nursing standards.
Ideal Candidate
We’re looking for an LPN who is reliable, clinically strong, compassionate, and comfortable working in a patient-focused LTC/Home Health environment.
Requirements
Active LPN/LVN license eligible to practice in Massachusetts.
Current BLS certification preferred/required per facility policy.
Recent LPN experience in LTC, Home Health, or a comparable setting strongly preferred.
Strong communication and clinical documentation skills.
Ability to work independently and as part of a multidisciplinary team.
Must be dependable and committed to the full 13-week assignment.
