*What You’ll Do*

\* Provide *mobile primary care* to geriatric patients within *25 miles of your home* \* Manage chronic conditions (*CHF, hypertension, diabetes, etc.*) to enhance quality of life \* Collaborate with a healthcare team to ensure *consistent, high-quality* patient care

*What We’re Looking For*

• ✔ *Licensed NP/PA-C* in Oregon
• ✔ *DEA License* (or ability to obtain within 90 days)
• ✔ *BLS/CPR Certification*
• ✔ Passion for *geriatric care* and chronic disease management
• ✔ *Reliable transportation & home office setup*
