Licensed Practical Nurse LPN | Personal Care Services
Part-Time Hourly | Branch: Kirksville, Missouri | Urgently Hiring
Territory: Moberly, MO and surrounding areas

As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:
Deliver in-home nursing care aligned with patient care plans
Review and follow Plan of Care prior to each visit
Communicate patient updates, concerns, and changes in condition
Assist with medication management and documentation
Educate patients and families on self-care techniques
Identify safety concerns and help prevent hospitalizations
Collaborate with RN Case Managers, physicians, and care team
What is required?
Current LPN/LVN license, in the state of Missouri
Graduate of an accredited nursing program
At least 1 year of experience in acute care (home care, home health or hospice preferred)
Strong communication and interpersonal skills
Ability to manage time effectively and stay organized in the field
Knowledge of healthcare regulations and patient care standards
Compassionate, patient-first mindset
Reliable personal transportation (travel required ~50%)
Ability to meet health screening and compliance requirements
You will report to the Clinical Manager or Branch Director for Personal Care Services.
